Position Overview: We are seeking a compassionate and skilled Physical Therapist to join our dynamic healthcare team. The ideal candidate will have a passion for helping patients regain their strength, mobility, and independence through personalized therapy programs.

Key Responsibilities:

• Assess patient needs by reviewing medical history and conducting physical evaluations.

• Develop individualized treatment plans aimed at achieving specific goals.

• Provide hands-on physical therapy treatments using exercises, manual therapy techniques, equipment, or modalities.

• Educate patients on therapeutic exercises to be performed independently between sessions.

• Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.

• Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive care plans.

• Maintain accurate documentation of patient assessments, treatments provided, progress notes, and discharge summaries in compliance with company policy.

Qualifications:

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program required; Doctorate preferred (DPT).

• Current state licensure as a Physical Therapist or eligibility for licensure upon hire.

• Proven experience in developing effective treatment plans tailored to individual patient needs.

• Strong communication skills with the ability to educate patients effectively about their conditions and treatments.

• Ability to work both independently and collaboratively within a multidisciplinary team environment.
