Housing Case Manager needed to deliver thorough case management support to individuals and families facing housing instability, and guide clients towards permanent housing and greater independence using a client-centered, Housing First methodology.
**Primary Duties:**
· Perform detailed evaluations of clients’ housing requirements, background, and service eligibility.
· Create customized housing strategies based on individual needs and choices.
· Connect clients with suitable organizational and community resources, such as healthcare, mental health services, and social programs.
· Assist in navigating the application process for rental assistance initiatives like Section 8.
· Collaborate with property owners and housing organizations to find affordable accommodation options.
· Keep consistent communication with clients through home visits or other means.
· Track progress toward their residential stability and self-reliance objectives.
· Ensure accurate documentation of client information using HMIS/BAS-Net or related systems.
· Provide emergency intervention support when necessary.
· Educate about tenant rights and obligations.
· Prepare timely reports along with case notes.
· Participate in training sessions, workshops, meetings.
**Requirements:**
· Bachelor’s degree in social work or similar discipline (or equivalent experience).
· At least two years working with at-risk groups such as those experiencing homelessness or substance issues.
· Solid grasp of the Housing First Approach Principles.
· Strong skills in communication. organization, problem-solving.
· Valid driver's license; reliable transportation.
· Experience using HMIS/BAS-Net system.
· Familiarity with local housing resources.
· Spanish-speaking a plus
