Community Health Representative
We are seeking a dedicated Community Health Representative to serve as a vital link between the community and healthcare providers. In this role, you will work directly with community members to improve access to healthcare services, promote healthy behaviors, and provide health education.
**Key Responsibilities:**
- Act as a liaison between clients and social service professionals.
- Conduct outreach activities in the community to raise awareness about available services.
- Assist clients in navigating the healthcare and social service system by scheduling appointments, arranging transportation, and completing necessary paperwork.
- Provide culturally appropriate health education on topics such as nutrition, disease prevention, maternal-child health care, immunizations, etc.
- Collaborate with local organizations to develop programs that address specific health needs within the community.
- Maintain accurate records of client interactions and track progress toward meeting individual goals.
**Qualifications:**
- Bachelor’s degree required.
- Previous experience working in a community-based setting or related field is highly desirable.
- Strong interpersonal skills with an ability to build trusting relationships within diverse communities.
- Excellent communication skills both verbal and written; bilingual abilities are a plus.
- Knowledge of local resources available for addressing social determinants of health (e.g., housing assistance).
