Title: Social Worker

Position Overview: We are seeking a dedicated Social Worker to support families in overcoming obstacles and accessing essential services. This role involves providing emotional guidance and assisting them in developing the skills and resilience needed for a better quality of life.

Key Responsibilities:

- **Needs Assessment:** Perform evaluations to determine the specific needs and objectives of families.
- **Case Coordination:** Create tailored support plans with ongoing review to track family progress.
- **Resource Navigation:** Connect families with community resources, advocating on their behalf to ensure service accessibility.
- **Emergency Support:** Offer immediate assistance during crises as required.
- **Group Facilitation:** Lead support groups, offering educational sessions on topics related to family well-being.

Additional Duties:

- Conduct home visits with expectant mothers and parents of young children for developmental assessments, prenatal advice, maternal health information, and parenting guidance.
- Keep precise records of cases with up-to-date documentation.

Required Qualifications:

- **Education:** Bachelor’s or Master’s degree in social work or a closely related discipline.
- **Experience:** Proven experience working with families experiencing crises is essential.
- **Skills:** Excellent interpersonal communication abilities, strong empathy skills; proficiency in Spanish is advantageous.
