Case Manager

Position: Case Manager

We are seeking several dedicated, full-time to deliver case management services. You'll play a critical role in crisis response, facilitating group sessions, and providing counseling support for single parents with limited income, pregnant women, new parents, and individuals transitioning from homemaker roles. You will work both within our centers and out in the community.

Key Responsibilities:

- Conduct client interviews during the intake phase to gather psychological, emotional, and social data.
- Collaborate with clients to develop action plans by identifying options and setting achievable objectives.
- Connect clients with community resources by coordinating appointments and building partnerships with other organizations.
- Encourage client development by analyzing behaviors, offering explanations, and suggesting alternative approaches.
- Keep detailed records of case activities.
- Regularly check-in on clients' progress and adapt goals as necessary.
- Comply with organizational policies while engaging in quality checks; suggest improvements when needed.
- Safeguard client confidentiality while protecting internal operations by maintaining privacy standards.
- Support team initiatives by assisting during organizational events.

Required Skills:

- Strong sense of objectivity, confidentiality commitment, and perseverance
- Effective planning coupled with strong organizational capabilities
- Superior listening skills along with excellent written and oral communication proficiency
- Empathy paired with strong interpersonal relations skills
-Proficiency using Microsoft Office suite
-Capable of working independently as well as collaboratively within a team

Education & Experience Requirements:

-A Bachelor’s degree in psychology or social work is required; related fields considered
-Master's degree in social work or licensed clinical social worker status is advantageous
-Possess 2–5 years of relevant professional experience

Spanish-speaking a plus.
