BONUS-Facility would like to offer a $2,000 completion bonus to the traveler after 16-weeks. Terms: 576 hours in a 16-week period with no more than 2 call offs or tardies. All Documentation must be completed in a timely manner and satisfactory

36 hours guaranteed with the expectation of working up to 40 hours.
OT must be approved by director before any OT hours are worked.
Productivity expectation is 60 visits/ week; Will see 12-14 patients/day; Double-booking of follow up visits expected.
