Responsibilities of the Licensed Practical Nurse (LPN):

• Administration of prescribed medication, internal feeding, and other treatments and therapies

• One to one patient care in a home environment

• Execution of physicians prescribed plan of care and compliant documentation of care provided in system of record

• Documenting shift reports, incident reports, and individual progress notes related to patient assessments and coordination of care

• Work closely and maintain daily communications with the Certified Nursing Assistants so that a continuity of care and services is maintained for the residents

• Other duties as assigned

Qualifications

Education:

• Licensed Practical Nurse (LPN) Florida License

• Certification in CPR and First Aid as required by the state

Experience:

• Licensed Practical Nurse with 3+ years experience

Available Shifts:

• (3) 12 hour OVERNIGHT shifts per week, 7pm-7:30am

• 2 Set shifts: Thursday & Friday,

• 1 Alternating shift Wednesday & Saturday

• Must be able to work some holidays.

Other Requirements:

• Background Screening

• Valid driver's license

• Must be able to communicate effectively with residents, families, associates, community, and state officials.

• Must be comfortable with computer use and have previous experience with computerized resident documentation systems a plus.

• The Community is a drug free workplace; associates are subject to adherence to the Company Drug Free Workplace policy.

Job Type: Full-time, 36 hours per week

Pay: $28.00 - $30.00 per hour
