Full Time Hospice Admissions Nurse (New Nurses encouraged to apply)

Duties Include:

• Completing initial and ongoing comprehensive assessments of patient and family to determine hospice needs.

• Provide professional nursing care.

• Initiate the plan of care and make necessary revisions as patient status and needs change.

• Develop a care plan that establishes goals, based on the nursing diagnosis and incorporates palliative nursing actions - includes the patient and the family in the planning process.

• Counsel the patient and family in meeting nursing and related needs.

• Meet weekly with IDT members

Requirements/Qualifications :

• NEW NURSES WELCOMED
• Graduate of an accredited school of nursing.

• Current licensure in State and valid driver’s license. * One year of hospice experience preferred.

• Excellent observation, verbal and written communication skills,
